The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Financial Policy

At Shaun Michael Lee DDS, we are committed to providing high-quality dental care while being transparent about financial responsibilities. Our goal is to make payment arrangements as simple and stress-free as possible.

Payment is due at the time of service unless other arrangements have been made in advance. We accept:

• Cash
• Debit and major credit cards (Visa, MasterCard, etc.)
• Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA)
• Third-party financing (e.g., CareCredit) for qualified patients

For patients with dental insurance:

• As a courtesy, we will file your dental claims and help estimate your benefits.
• Please note that insurance companies do not guarantee payment—even when a procedure is listed as a covered benefit.
• If your insurance does not pay as expected, any remaining balance becomes the patient’s responsibility and will be billed accordingly.
• Co-pays, deductibles, and non-covered services are due at the time of treatment.

For uninsured patients:

We offer transparent pricing and flexible payment options to help make dental care more accessible.

Missed Appointments and Cancellations:

We kindly ask for at least 24 hours’ notice for cancellations or changes. Missed or late-cancelled appointments may be subject to a fee.

Dental Insurance

At Shaun Michael Lee DDS, we are committed to helping you maximize your insurance benefits. While dental insurance can help reduce your out-of-pocket expenses, please note that coverage varies greatly between plans and providers.

We accept most PPO dental insurance plans and are happy to assist you in understanding and verifying your coverage. As a courtesy, our office will submit claims on your behalf and provide you with an estimate of your benefits prior to treatment.

Please keep in mind:

• Your insurance policy is a contract between you and your insurance company.
• All co-pays, deductibles, and any non-covered procedures are due at the time of service.
• Final responsibility for payment lies with the patient if insurance does not pay as expected.

Not insured? We also offer flexible payment options and discounts for patients without insurance. Please contact our office for insurance details. We look forward to hearing from you.
